It usually indicate kidney damage, usually by underlying disease such as :

• High blood pressure
• Obesity
• metabolic syndrome
• Genetic inherited kidney diseases
• Type I and type II diabetes
• infections

Usually starts by modifying life style, limiting sugar in the diet,  treating the underlying conditions and some medications such as Angiotensin-converting enzyme inhibitors  and Angiotensin II receptor blockers and treating infections
