Blood in the sperm analysis 
The presence of painless blood in the sperm analysis might indicate active inflammatory process due to bacterial infection in the testis the cause pus cells and RBCs to present in the sperm analysis, additional causes are testicular or prostatic tumors that should be investigated to define the appropriate management after consultation of a urologist 
Investigations 
After proper physical exam , cbc should be done to  detect anemia, Semen analysis to look for signs of infection, prostate recal exam should be done in addition to Testicular and abdominal US to look for testicular and prostatic tumors, PSA level for prostate tumors, Pelvic MRI to look for signs of prostate tumors
