Good day, my wife has been experiencing a pain while passing her stool since the last year.. she has tried many remedies.. she used to do stiz regularly and was given a potassium permanganate solution for the sitz.. also a betnovate cream which didn't help much.. after that didn't work, we tried to just change her diet to healthy and non spicy foodstuffs which would give a smooth stool, however if she eats something mildly spicy, or any fried food she experiences burning due to the area being extremely sensitive.. And hard stools which cause a lot of pain... There is no external wound, but there is swelling 90% of the time.. we do not want any numbing cream or pain killers, just something that will heal the skin faster and decrease the soreness in the area...

what is the treatment of painful defecation ?
the treatment of such cases is depending upon the cause of the probem so in case of anal fissure ( and its not nessesery for you to see them because there are internal ones that cannt be seen but under general anesthesia and anal dialator ) the treatment is with nitrate cream or via surgical means, in case of ananl internal biles alson this can be treated surgically and in other cases the treatment is by an antiparasitic agents such as mebendazole .   
what are the diagnosit tests for the painful defecation ?
at first a past medical history and clincal examination, identifng patents own sexaul habits and sexual partnet health for std, ana swap to check if there is an std( sexually transmitted diseases ), anscope to check for fissures or biles, stool analysis in case of parasitic infection is suspected and in some cases a colonoscpy is indicated 
what are the possible causes ?
  • biles (hemorrhoids) 
  • anal cracks or fissures 
  • anal or rectal cancer 
  • parasitic infection 
  • anal infection
