When we taik about diffculty of defacating with things go out and come back , we go to diagonse hemorrhoids which cause due to increase in intra-abdominal pressure such as recurrant constipation , recuurant cough and elevate heavy things ,  which come as descriped above some veins go out then come back , there is four degrees of hemorrhoids will clarify below , managment according this degree and by avoid increase in intra-abdominal pressure by avoid constioation by eating food rich with fibers as banana and increase drink of water and liquids , Type (1+2+3 ) can treat it medically by avoid icrease intta-abdominal pressure and give sitz baths , if go to type 4 you should treat it surgically ,

Advice you to go to general surgery doctor who will take complete history , do examination and some investigations then follow your case 
: Types of hemorrhoids and managment 

• Type 1 : which come with pain and some blood in stool without appear anything  
• type 2 : which come with veins go out and come back immediatlly
• Type 3 : which contain veins go out and come back manually by patient himself 
• Type 4 : Go out and do not come back and patient can not get it back
