30 aged male patient with uncontrolled diabetes do a surgical gastric bypass , after 22 days the wounds after surgery is not recovered yet as it swallowed and it discharge a yellow water and the blood sugar level mainly high 200-300 all the time , he taken fast acting insulin 3 times daily before every meal and long acting insulin at night , he diagnosed by COVID-19 after 2 weeks from the surgery , he has edema in both legs and he is tired , the oxygen saturation mainly 85 but he is tired and loss his appetite after the surgery , loss fluids , hemoglobin 9.5 doctor give him a IM lasix ampule / 24 hour is that right ? And what the suitable treatment and management for this case , plz the is very critical and need answer immediately

Treatment of patient:
According to your description, your patient mostly suffer from spesis ( infection ), and it is mostly due to surgical site infection, so your patient needs immediate proper treatment that may include urgent surgical intervention to clean pus and explore abdomen, and he will need to be admitted to ICU for close monitoring and treatment with fluids, antibiotics, oxygen...etc
Diagnosis of the cause:
It depends on the clinical examination by surgeon and investigations as CBC to detect infection, Abdomen US, x-ray and CT scan, renal and liver functions, urine analysis, ABGs, chest x-ray ...etc
Symptoms of sepsis:
- General fatigue
- Fever
- Decreas saturation
- Tachypnea
- Tachycardia
- Dyspnea...etc
