Hello doctor Left iliac bone focal pathological osseous lesion is seen measuring 3.2x2x4.2cm at its maximal AP, TS and CC dimensions respectively .it is seen adjacent to the upper part of the left sacroiliac joint. It elicits low T1 signal with heterogeneous mixed signal in PD and STIR Wis with marginal sclerotic changes. It shows mild contrast uptake in post contrast scan.no associated cortical disruption, periosteal reaction of soft tissue components.  Otherwise normal bone marrow signal of the other scanned bones  Intact pelvic and the scanned upper muscles without areas of abnormal signal or masses. Intact intermuscular fascial planes with no fluid collections seen  Intact both hip joints with preserved spherical contour of both femoral heads with no avascular necrosis  No pelvic masses or collections.  Smooth filling of the urinary bladder with no mural thickening or diverticular outpouching. Conclusion:  Left iliac bone focal pathological marginally sclerotic osseous lesion with signal criteria and contrast pattern as detailed without associated cortical disruption, periosteal reaction of soft tissue components, for better further histopathological assessment by bone biopsy

Based on your question


There is left iliac lesion with the mentioned dimension. Other part of your report is normal as there is other lesion , masses or tissues reactions or inflammation. Like lesion and for better evaluation can be removed surgically and referred for pathological biopsy and intervention to determine what is it for proper evaluation and treatment

Investigations to do


As we said, it is better to removed surgically for pathological biopsy 

Causes


There are many causes like
  • Calcification only.
  • Bone reaction 
  • Bone benign tumors
  • Malignant tumors in iliac bone
  • Other causes

Thanks for your reply . But before has to confirm if there is mass and whether it is benign or malignant ? If benign it is not mandatory to be removed right?I’ll share the MRI images from my email hussein.baradie@gmail.comThanks
