What is the treatment 
The described sympomtes of testicular pain and penile pimples represent a concurrent bacterial infection that will cause testicular inflammation and should be treated using Doxacyclin 500 mg tab twice daily for 10 days in addition to the use of Penicillin G tab 4 times a day for 5 days. Consult a urologist for follow up.
Investigations 
After proper physical exam, cbc should be done to look for signs of infection , Semen analysis to look for bacteria in the semen and testicular US to look for signs of inflammation

Bacterial infection

Viral orchitis
